OFFICE POLICIES

We have developed new office protocols in response to the COVID-19 pandemic.  
• Our office asks that you wear a mask in our office if you have a cough, fever, or other symptoms of a viral illness.
• Please follow social distancing guidelines when in the office.
• Please limit the number of people accompanying you to your appointment as much as you can.

Emergencies: If you have a life threatening emergency, please call 911. Otherwise, please contact our office. If you call during our office hours, we will care for you appropriately. If you call after hours, you will be directed to call our answering service, and we have a physician on call 24 hours a day, 7 days a week.

Cancellation Policy: We make every attempt to contact you two days before your appointment with a reminder call. We ask that you provide us with the courtesy of canceling your appointment if you are unable to keep it. We reserve the right to charge for a missed appointment.
Medication Refills: If you need a medication refill, please call your pharmacy, and they will call us. Occasionally you will need an appointment or lab work before we refill your medication, and we will inform you if this is the case. We do not refill narcotics after hours or on the weekend. Please provide us with 72 hours notice for all prescription refill requests; this will allow for any delays due to staffing, high request volume, or office closures.
Billing and Payment: We participate with most insurance plans. We ask that you bring your most recent copy of your insurance card with you to every visit to ensure that we can file your claim. Please remember that it is your responsibility to know the coverage and limits of your insurance plan, we cannot quote you benefits. We ask that you pay your co-payment at the time of service. You will also be responsible for any deductibles and non-covered services. If you do not have insurance, we will be happy to see you and we expect payment at the time of service. No Surprises Act: In compliance with the No Surprises Act, we will provide a good faith estimate to you if we are out of network with your insurance company, if you do not have insurance, or if you otherwise qualify to receive a good faith estimate.  Please contact our office at 303-789-2251 for further information or to request a good faith estimate prior to your appointment.
